Before scheduling an appointment to be seen at the clinics, uninsured individuals will first go through a registration process to determine eligibility.  This essential step ensures that MobileMed can offer you the comprehensive healthcare services you need.  Please click below for the list of accepted documents.
 
Please remember to bring the following documents to your appointment:
  • Photo ID
  • Proof of Address: A recent phone bill, utility bill, or any official document displaying your name and address.
  • Proof of Income: Two recent pay stubs or a copy of your income tax return.
  • Medicare or Medicaid Card: If you are currently enrolled
