Mobile Medical Care, Inc., a Federally Qualified Health Center located in Montgomery County, Maryland serving low-income adults, is seeking a part-time Behavioral Health Case Manager (20 hours per week). The candidate must be familiar with Maryland laws and Montgomery County, Maryland regulations.
This is an exciting and challenging position in one of the oldest and most respected safety net health clinics in Maryland. This position is for the individual who is interested in providing quality customer service to our patient population who come from over 100 countries.
The position of Behavioral Health Case Manager connects Mobile Medical Care, Inc. (MobileMed) patients to behavioral health and substance use treatment at a higher level of care than MobileMed can provide in an integrated primary care setting. In addition, the Behavioral Health Case Manager will connect MobileMed patients to additional resources in the community as needed to support basic needs, including personal and professional development.

Job Responsibilities:
- Work under the guidance of the Behavioral Health Program Manager and Clinical Director to provide case management to Mobile Med patients needing a higher level of behavioral health and substance use care than a primary care setting provides.
- Track internal referrals assigned to you and follow up with referred patients in a timely manner using documentation in the electronic medical record.
- Use SBIRT model to identify substance use concerns in MobileMed patients and encourage patients to connect to substance use resources and treatment at an appropriate level of care.
- Identify level of mental health concerns in MobileMed patients and support connection to services at appropriate level of care.
- Assess for safety with patients in crisis or experiencing acute symptoms and connect to emergency behavioral health services when appropriate.
- Assist patients with access to basic needs such as identifying resources for food, housing, and vocational support and helping patients connect and utilize such resources.
- Coordinate care with external agencies that provide services to Mobile Med patients including but not limited to the provision of basic needs, vocational support, behavioral health and substance use services.
- Collaborate with PCPs, the BH team, and consulting psychiatrist to ensure coordination of care and achievement of good clinical outcomes. Inform the Behavioral Health Program Manager and/or Mobile Med Clinical Director in cases of patient risk of harm to self or others.
- Follow standard medical and behavioral health procedures to protect patient privacy including HIPPA standards, medical release of records and encrypted forms of communication.
- Ensure proper case documentation in electronic health record system.
Qualifications/Competencies:
The position requires a Bachelor's Degree in Social Work or comparable experience, and a proven ability to meet goals and objectives with minimal supervision. Competency in the following areas or capacity to learn and put into practice the skills listed below.
- Social work, 1 year (Preferred)
- Strong interpersonal and communication skills utilizing sensitivity to multicultural issues.
- Case management skills including knowledge of local resources for mental health, substance use and basic needs; proper case documentation in MobileMed’s electronic health record system; use of Microsoft 365 Teams, Outlook, OneDrive, Word and Excel including ability to create and maintain tracking spreadsheet.
- Bilingual Spanish-speaking is required.
- Must be fully vaccinated against COVID-19.
- Willing to undergo a background and drug test prior to employment.

The position of Behavioral Health Therapist is a 20-30 hour per week job.
The position of Behavioral Health Therapist helps ensure that MobileMed patients receive behavioral health interventions and therapy designed to address behavioral health needs, as appropriate to an integrated primary care setting. The BH Therapist provides medically necessary and appropriate behavioral health services to individuals, groups, and families, within the scope of their training and experience. These services shall comply with legal requirements, accepted clinical standards, internal procedures, insurance requirements, and the professional and ethical standards by which mental health therapists are bound.
CANDIDATES MUST BE BI-LINGUAL IN ENGLISH AND SPANISH.
Job Responsibilities:
- Provide clinical services appropriate to a primary care setting.
- Collaborate with PCPs, the BH patient care monitor, and consulting psychiatrist to ensure coordination of care and achievement of good clinical outcomes.
- Provide support to integrated behavioral health volunteers and students.
- Ensure proper case documentation in electronic health record system.
- Promptly notify Program Manager for BH of crisis situations. Seek clinical supervision and consulting with colleagues when appropriate.
- Conduct a regular weekly schedule.
- Maintain professional license in good standing.
Qualifications/Competencies:
The position requires a Masters level degree, licensure as a mental health therapist in the State of Maryland, and a proven ability to meet goals and objectives with minimal supervision.
- Strong therapeutic clinical and case management skills.
- Sensitivity to multicultural issues.
- Strong interpersonal skills.
- Tenacity to work with a primary care/behavioral health team to resolve programmatic issues.
- Ability to monitor patient outcomes.
- Ability to promptly, accurately document billable patient services so that third-party reimbursement can be obtained.
- Bi-lingual Spanish-speaking required.

Duties:
- Conduct initial assessment of patients, including checking vital signs, height, weight, and chief complaint.
- Conduct other components of nursing assessment under the direction of the RN Nurse Manager.
- Maintain, manage and keep confidential the electronic medical record for assigned clinic sessions.
- Conduct intake and registration of new and returning patients. Including all point of care testing and screening.
- In coordination with other team members, enter patient visit data into the electronic medical record.
- Make patient appointments, using the electronic appointment calendar.
- In collaboration with the clinic RN Nurse Manager, participate in case management and navigation activities.
- Participate in Quality Improvement activities as assigned.
- Complete EKG’s and nebulization treatments as ordered.
- Complete ear irrigation under the direction of the RN Nurse Manager.
- Assist in pelvic exams and office procedures as assigned.
- Provide interpretation as requested.
- Provide phlebotomy and injections as ordered.
- Delivers specimens as required to Adventist Healthcare.
- Trains other MA’s as needed.
- Provide excellent customer service.
- Complete incident reports when necessary.
- Maintain excellent time and attendance.
- Assignments may be changed or adjusted as necessary for operations. This individual will accommodate these changes.
- Front desk duties as assigned.
- Foster a sense of health and safety in the clinic and alert management to any problems, including, broken medical equipment, environmental safety concerns and expired medication or supplies.
- Other duties as assigned.
Qualifications:
- Formal preparation and certification as a Medical Assistant, with excellent technical skills.
- Acts appropriately with a multi-cultural population.
- Bilingual in English/Spanish or English/French.
- Proficiency in documenting in an electronic health system.
- Basic computer skills, keyboarding and ability to perform basic operations with the database.
- Must provide proof of full COVID-19 vaccination.
- Well organized, able to prioritize and meet deadlines.
