Mobile Medical Care, Inc. (MobileMed) is a Federally Qualified Health Center and nonprofit community healthcare organization serving low-income adults in Montgomery County, Maryland. MobileMed’s practice environment is collaborative and patient-centered.

Description:

Responsible for the smooth and efficient operation of services that contribute to the overall productivity and quality of care provided by MobileMed clinics, while maintaining compliance with applicable regulations and policies.

Duties:

Clinical Oversight

  • Must possess high-level assessment skills that are needed to determine patient’s well-being and current status. Patients may require intervention by the RN to determine their need for immediate care. RN needs to be comfortable with physical assessment skills and the ability to assess patients for serious illness, especially in populations whose first language is other than English.
  • RN disseminates this knowledge to MA’s and other personnel as needed during her tenure.

Case Manager

  • Follows up with Specialty Referral Coordinator, Catholic Charities and others to insure timely and accurate specialty referrals.
  • Acts as liaison with local hospital staff, including emergency, laboratory, and radiology departments, as related to the provision of care in assigned clinic(s).
  • Must follow up on patient visits and assure patient compliance with referrals and diagnostic testing and acts on all critical or abnormal tests as appropriate.
  • Must recognize that MobileMed’s complex patient population requires significant attention to patients who are not familiar with navigating the healthcare system. Extra attention will need to be placed on making sure patients understand the process.
  • Works with the Transition of Care Coordinator (TCC) to insure appropriate information is present for the patient’s visit and to provide the TCC with information for reporting purposes.

Supervisor

  • Supervises the Medical Assistant and Registrar in day to day operations, this includes, discharge planning, medication distribution, appointment set up, application processing, co-pay collection, depositing money, and patient education.
  • Must understand MobileMed’s Policies and Procedures.
  • Attends All-Staff Meetings and Nurse/Provider Meetings as scheduled.
  • Brings any personnel or clinic issues to the attention of the Clinical Director at once.

Communication, Documentation, Organization and Flow

  • Provides oversight of data entry into eCW electronic health record system.
  • Reviews provider notes, including orders, codes and follow up appointments.
  • Communicates with volunteer providers regarding scheduling, documentation, referrals and medications.
  • Responsible for inventory of medical and office supplies and medication.
  • Communicates with front office personnel regarding supplies, medications or any scheduling concerns.
  • Is responsible for the clinic flow. Approving walk-ins and making assignment changes as necessary to improve volume when possible.
  • Participates in team activities as directed.
  • Responsible for safe handling and scheduling pick-up of bio-hazardous waste l in assigned clinic(s), and compliance with applicable OSHA standards.
  • Makes appointments when necessary.
  • Communicates with volunteer providers regarding scheduling, documentation, referrals and medications.
  • Broken equipment should be isolated and identified and the Assistant Clinical Director is notified by email.
Qualifications:
  • Registered nurse in the State of Maryland.
  • Ability to interact with multicultural population and providers.
  • Excellent organizational skills.
  • Experience in not-for-profit healthcare organization a plus.
  • Electronic health record experience a plus.
  • Current CPR certification for healthcare workers.
  • Fluency in English, required.
  • Other languages, especially Spanish, preferred.

Reports to and under the supervision of the Clinical Director.
